What can a Clinical Pharmacist do?

A clinical pharmacist’s role is to optimise medication and patient outcomes; they can review your prescribed medication but can’t prescribe new medication. Any new medication is required to be prescribed by your GP or NP.

Seeing a clinical pharmacist helps you get the best from your medications while ensuring they’re safe, effective, and right for your needs. A clinical pharmacist can:

  • Identify and resolve issues early, such as side effects or interactions
  • Provide clear guidance on how and when to take your medications
  • Support you in managing long-term conditions or starting new treatments
  • Work closely with your doctor and is available for appointments to review your medications or discuss any concerns

If you require additional information or have any questions about your prescriptions, a clinical pharmacist provides clarity on your treatment plan and can explain how to take your medications appropriately.

During hospital admissions or discharge, clinical pharmacists can help with your medication management.
